Physiotherapy in Your Clayton Setting

Clayton Community Centre is on Cooke Street, and Monash Medical Centre is on Clayton Road. These are local reference points, not partner services. If you have relevant discharge information or an existing rehabilitation plan, you can share it so we understand your current care.

Clayton's mix of houses and apartments can mean very different entrances and living spaces. We assess your own access, furniture and equipment rather than assuming the same home program suits every address.

Our sessions are home or community visits. We set up programs with your own equipment, steps and furniture, with tasks planned around your NDIS goals.

How our home-visit physiotherapy works →

Neurological Rehabilitation

Neurological rehabilitation can address changes in coordination, walking and transfers after stroke or with a long-term neurological condition. We link practice to the activities you want to manage in your home and community.

Falls & Balance

We explore balance during getting up, turning and walking, alongside the circumstances of any falls. The plan may include practising a doorway or discussing how you use an aid around the house.

Strength & Capacity Building

Strength and capacity work can target repeated chair rises, standing tolerance and the stamina needed for household routines. Exercise dosage is agreed with you and adjusted around fatigue and existing health advice.

Mobility & Equipment

Mobility assessments consider walking aids, transfers and the space available in your home. We can document recommendations and, with consent, communicate with your support coordinator or existing allied health team.
