Adult NDIS Home Visits · South-east Melbourne

Physio at home in Hallam, planned around your NDIS goals.

We bring adult NDIS physiotherapy to homes in Hallam, with care shaped around the movement tasks you use each day. Rather than asking you to recreate your home routine in a clinic, we look at how you get around your own rooms, entrances and outdoor spaces.

For NDIS self-managed and plan-managed participants.

Physiotherapy in Your Hallam Setting

Hallam sits between Doveton, Hampton Park and Narre Warren, with Hallam railway station serving the suburb. A goal might involve managing the front path, getting into a vehicle or building the walking tolerance needed for a local errand; we start with what is relevant to you.

Home visits let us consider the actual chair heights, steps and room layout you use. We can involve a family member or support worker, with your consent, so practice between visits follows an agreed approach.

Our sessions are home or community visits. We set up programs with your own equipment, steps and furniture, with tasks planned around your NDIS goals.

How our home-visit physiotherapy works →

Neurological Rehabilitation

For adults living with stroke, Parkinson's disease or another neurological condition, we work on movement control, walking and everyday transfers. Tasks are broken into manageable steps using your own furniture and usual support arrangements.

Falls & Balance

Balance work can include turning in a hallway, stepping over an entrance threshold and moving between indoor and outdoor surfaces. We discuss falls concerns and consider how a walking aid fits your daily routine.

Strength & Capacity Building

Capacity-building exercises can focus on rising from a chair, standing for household tasks and pacing activity. We adapt the starting level and home program to your abilities, fatigue and NDIS goals.

Mobility & Equipment

We assess walking aids, transfers and home set-up in the spaces where you use them. Home set-up discussions can include the route between the bedroom and bathroom, space for assistance and practical equipment recommendations.
